Responsibilities
- Coordinate patient care plans and treatment schedules during weekend shifts
- Collaborate with multidisciplinary teams to ensure continuity of care
- Document patient interactions and maintain accurate medical records
- Respond to urgent patient inquiries and escalate critical issues promptly
- Conduct weekend wellness checks and monitor patient recovery progress
- Assist in weekend admissions and discharge procedures

Qualifications
- Associate's degree in Healthcare Administration or related field
- 2+ years of healthcare coordination experience
- Current California healthcare certification (CNA, LVN, or equivalent)
