Responsibilities
- Provide direct patient care assistance including mobility support and vital sign monitoring
- Maintain accurate medical documentation and electronic health records
- Coordinate with multidisciplinary teams to ensure seamless patient transitions
- Respond promptly to patient needs and emergency situations per protocols
- Assist with patient education and discharge planning procedures
- Ensure compliance with HIPAA and facility safety regulations
Qualifications
- High school diploma or equivalent (CNA certification preferred)
- Basic Life Support (BLS) certification required (training available)
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
- Flexibility to work Saturdays and Sundays (6am-6pm shifts)
