Responsibilities
- Provide direct patient care including vital sign monitoring, medication administration, and mobility assistance
- Collaborate with interdisciplinary teams to ensure seamless patient transitions
- Maintain accurate electronic health records and documentation
- Respond to patient emergencies following established protocols
- Assist with patient admissions, discharges, and transfers
- Participate in weekend shift huddles and care coordination meetings
Qualifications
- Current Oklahoma LPN or CNA license required
- Minimum 1 year acute care experience preferred
- BLS certification with current provider status
- Ability to work independently and in team settings
- Strong communication and interpersonal skills
- Flexibility to work rotating weekends and holidays
- Basic computer proficiency with EMR systems
