Responsibilities
- Assist patients with daily living activities including bathing, dressing, and mobility support
- Monitor vital signs and document patient observations accurately in electronic health records
- Collaborate with nursing staff to implement personalized care plans
- Maintain a clean, safe, and organized patient environment
- Provide emotional support and companionship to patients during weekends
- Respond promptly to patient calls and emergency situations
- Coordinate with interdisciplinary teams for seamless care transitions
Qualifications
- High school diploma or equivalent required; CNA certification preferred
- Minimum 1 year healthcare experience in acute or long-term care setting
- Current BLS/CPR certification from American Heart Association
- Strong interpersonal skills with ability to work sensitively with diverse populations
- Ability to stand/walk for extended periods and lift up to 50 pounds
- Proficient in using electronic health record systems
- Flexibility to work weekends (Sat/Sun) with potential holiday coverage
