Responsibilities
- Provide direct patient care including vital monitoring, medication administration, and assistance with daily activities
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Document patient observations and care activities accurately in electronic health records
- Respond promptly to patient needs and emergency situations following protocols
- Maintain a clean, safe, and compliant patient environment
- Participate in shift handovers and team huddles to ensure continuity of care
- Support patient education and discharge planning processes
Qualifications
- Active CNA certification or equivalent healthcare credential required
- Minimum 1 year experience in clinical or long-term care setting
- Ability to work weekends and nights with reliable transportation
- Current CPR/BLS certification or willingness to obtain immediately
- Strong communication skills and ability to work independently
- Basic computer proficiency for documentation systems
- Must pass background check and pre-employment drug screening
