Responsibilities
- Coordinate patient care plans and treatment schedules during weekend shifts
- Collaborate with multidisciplinary teams to ensure seamless patient transitions
- Monitor patient vitals and document health progress using EMR systems
- Provide patient education and emotional support to individuals and families
- Manage weekend staffing assignments and departmental workflows
- Participate in weekend quality improvement initiatives
- Respond to emergency situations following established protocols
Qualifications
- Associate's degree in healthcare or related field (Bachelor's preferred)
- Minimum 2 years clinical experience in a hospital or clinic setting
- Current Wisconsin nursing or medical assistant certification
- Proficiency with electronic medical records (Epic experience a plus)
- Strong communication and conflict resolution skills
- Ability to work independently and make critical decisions
- Flexibility to adapt to changing weekend schedules
- Valid Wisconsin driver's license for occasional travel
