Responsibilities
- Coordinate patient care plans and weekend treatment schedules
- Provide direct patient support and family education during weekend shifts
- Collaborate with interdisciplinary teams to ensure continuity of care
- Manage electronic health records and documentation with precision
- Respond to patient inquiries and emergencies promptly
- Maintain HIPAA compliance and strict confidentiality standards
- Participate in weekend staff meetings and quality improvement initiatives
Qualifications
- Associate's degree in Health Science or related field (or equivalent experience)
- 1+ years of health care coordination experience
- Valid Texas state health care certification
- Strong communication and conflict resolution skills
- Ability to work independently during weekend shifts
- Proficient in EMR systems (Epic preferred)
- Flexibility to cover rotating weekends (Saturday/Sunday)
- Spanish bilingual proficiency required
